A Community Health Worker (CHW) bridges healthcare providers and their communities, improving health outcomes through culturally relevant education, outreach, and advocacy. CHWs help individuals navigate healthcare and social services, connect them to essential resources, and address barriers like language, transportation, and medical literacy. CHWs also promote preventive care, support chronic disease management, and assist with public health initiatives, fostering healthier, more resilient communities.
